A theatre nurse, having a tool that could have saved the life of a patient undergoing surgery, refrained from speaking up. A flyover, a section of a monorail, and a crane designed and built by well-qualified and experienced engineers all collapsed. A pilot, aware the aircraft was unserviceable for the flight, decided not to speak up, ending up dying in a crash shortly after take-off.
